To make an appointment please click the link above then select PBC State High School Appointment. Choose from the 8 available time slots between 13:30 - 15:30 on Mondays. 

Once you select a time you will receive an email with an intake form. Complete the intake form and make sure you include your parent / guadian's email and phone number in the 'add contact' section of the form. A consent form will also be sent to your parent / guardian to sign. 

*Please note that both the intake and consent forms will need to be completed prior to your appointment.
